NHS trusts now imposing cruel new pain tests for hip op | Daily Mail Online
[significant impairment as w transgender healthcare] Op 'rationing'
The Daily Mail says strict rationing rules being used by three quarters of NHS hospitals means patients in need of a replacement knee or hip must have to use a walking stick or wheelchair before they will be considered for surgery.

The paper says patients can be rejected for surgery unless they are judged to be in distress for most of the day, unable to sleep, or continually taking painkillers.

It adds that patients who are told they can't have an operation are advised to install hand rails at home, perform stretches or buy special shoes.

NHS slams big pharma in High Court row over drug pricing powers
The row is over powers introduced in April giving NHS England the right to ration costly medicines, including if they are expected to cost more than £20m in any of their first three years of use. Previously medicines signed off as clinically effective and good value for money by public drug price regulator Nice had to be automatically funded and made available by the NHS within three months. [...] The drugmakers argue the changes will limit patients’ access to cutting-edge treatments, particularly for rare diseases where the benefits are large but the patient numbers are small. In their claim the drug firms argue Nice acted beyond its powers to introduce the so-called budget impact test and failed to consult properly with industry on the detail of the proposals.

If our government won’t act to save our NHS this winter, this is what we must do | openDemocracy
Theresa May’s team has adopted an ostrich-style response to the rising cash crisis and its likely impact this winter. But it’s clear that many of her MPs, fearing they could lose their reduced majorities at the next election, are pressing hard behind the scenes for a reprieve for local services.

MPs have already forced significant retreats from hospital downgrades in north Devon and Essex. Cabinet minister Andrea Leadsom has backed calls for a reprieve for hospital services at Banbury’s Horton General, and may yet have something to say about Oxfordshire’s latest bed closures.

Back to the future on inflation
[ no rising demand = no inflation ] Globalisation and the march of technology have strengthened the hand of employers and weakened that of workers. So it is no surprise that though unemployment has relentlessly fallen, wage growth remains in the doldrums.

Then there is the war between generations. As the baby boomers approached retirement, they sought to safeguard their wealth. Inflation-targeting central banks, the dismantling of the welfare state and fiscal austerity were the natural results. Are Britons falling out of love with booze? - BBC News
One of the problems with the ONS survey is that it is based on people's own recollections - and it is well known that we are terrible for underestimating our drinking.
But there is evidence to be found elsewhere that the fall in alcohol consumption is real.
The findings chime with a much longer running source - the British Beer and Pub Association's data on the sales of pure alcohol.
It shows in the early 1950s alcohol consumption per head was below four litres of pure alcohol a year - the equivalent of about 140 pints of beer.
In the following decades, that climbed, particularly during the 1960s and 1970s.
By 2004, it had peaked, topping 9.5 litres.
Since then though, it has fallen and is now back under eight litres.

Budget 2017: Hammond to tell us the Brexit vote could have been worse | Business | The Guardian
consumers carried on spending and businesses continued to expand.

The picture for the public finances is also looking rosier compared with Hammond’s maiden autumn statement in November. [...] But here’s what the chancellor will not tell you: living costs have gone up and will continue to do so. The poorest will be hit hardest as the cost of essentials such as food and heating take a growing chunk out of already tight family budgets. Wages will struggle to match rises in inflation and the government’s benefit cuts mean that incomes will fall for the poorest people in Britain. In short, unless the government does something significant to improve living standards this week, inequality will start rising again.

Leaders of 33,000 doctors urge PM to recognise that hospitals are ‘paralysed’ by soaring demand and warn of staff exodus [...] In a letter to May, the Royal College of Physicians (RCP) and a host of senior doctors told her that the NHS is trying to tackle so many problems with such limited resources that it is in danger of not being able to do its job of caring for patients. [...] >>> under pressure like never before in its 69-year history [...] investment levels are not sufficient to meet current or future patient needs. <<< [...] May has told Simon Stevens, the chief executive of NHS England, that the service will not get any extra money and that he should concentrate on making major efficiency savings. ///&! NHS boards face cuts totalling £407m (From Herald Scotland)
Scottish health boards are facing up to budget cuts on a scale never achieved before, and some are proposing savings deemed "high risk".
Figures on cuts planned by Scotland's 14 regional health boards and seven specialist boards showed total savin of £407m to be achieved this year. Critics said this would wipe out in a single year the bulk of the SNP's manifesto pledge to invest an extra £500 million in the NHS over the course of the next parliament. //^&! Private providers in the NHS - the debate reignites - BBC News
[ sending people private to cut waiting lists and time to meet targets. no targets mean no one has to be send to private hospitals for treatment means less expense at the current account year. & Not to call out the truth, diet-related causes, is Political. ] ... the use of private health provision had increased to 7.6% in 2015/16. This compares with a figure of 4.9% in 2010/11, the first year of the coalition government. This is the basis of Labour's claim of a big increase in privatisation under the Conservatives. Government sources, though, say there was a significant rise in under Labour when private hospitals were first used to help cut waiting lists. Back in 2006/07, the use of private health provision was just 2.8%.

NHS chiefs urge GP surgeries to carry out life-changing cataract operations | Daily Mail Online
The NHS only has capacity to do 330,000 cataract operations a year but this could be hugely increased if GP surgeries also offered them.

The guidance will be issued to England's 211 Clinical Commissioning Groups, which control local NHS budgets and decide which patients deserve treatment.

The health watchdog NICE is also drawing up guidelines on cataract treatment that are due in 2018.

Around half of over-65s suffer from cataracts to some degree. This occurs when the lens in front of the eye becomes cloudy with age.

But it can be easily treated with a quick operation to replace the affected lens with a plastic implant costing £800 an eye.

Lady Altmann, who lost her post in Theresa May's reshuffle, argued instead for a system whereby the state pension increased in line with either prices or earnings, but with no commitment to increase it by 2.5%.

Here's Why All Pension Funds Are Doomed, Doomed, Doomed
Given that both The Economist and The Wall Street Journal have covered the impossibility of pension funds achieving their expected returns, this reality cannot be a surprise to anyone in a leadership role. [...] Here's problem #1 in a nutshell: the average public pension fund still expects to earn an average annual return of 7.69%, year after year, decade after decade.

This is roughly triple the nominal (not adjusted for inflation) yield on a 30-year Treasury bond (about 2.65%). The only way any fund manager can earn 7.7% or more in a low-yield environment is to make extremely high risk bets that consistently pay off. //&! NHS budget control 'will require staff cuts'
It says the government should be honest about NHS spending plans at a time when patient demand is rising. [...] The NHS leadership is set to unveil plans to get hospitals and other trusts to stick to agreed spending limits. In the last financial year, trusts ran up a collective deficit of £2.4bn - and the aim is to cut that figure significantly. [...] "It is no longer credible to argue that the NHS can continue to meet increasing demand for services, deliver current standards of care and stay within its budget. This is widely understood within the NHS and now needs to be debated with the public. "There are no easy choices, but it would be disastrous to adopt a mindset that fails to acknowledge the serious state of the NHS in England today." //&! bbc.in/29mkr7G - An overspend would be a serious breach of Whitehall protocol and lead to senior officials being hauled over the coals by the National Audit Office.

Does The Myth of Meritocracy show us how to alleviate inequality?
No debate about social mobility is complete without reference to the supposedly halcyon days of the 1950s and 1960s. The class fluidity of those decades, however, was not the result of professions becoming more meritocratic. It was caused by an unprecedented rise in the number of middle-class jobs: there was more room at the top. [...] Bloodworth writes that 21st-century society is characterised by a dwindling of professional jobs and more people slipping downwards: there is “more room at the bottom”. [...] Globalisation has created what the economist Robert H Frank calls “winner-takes-all” markets

Annuity rates 'in freefall' since EU vote
Rates have fallen by 3.5% in the two weeks since 24 June, said Tom McPhail, head of retirement policy at Hargreaves Lansdown.
Before the vote, a 65 year-old with savings of £100,000 would have been able to buy an annual income of £5,069.
Now the value of that pension has dropped to £4,890, a new record low.
Experts had predicted just such a development before the referendum, yet a majority of people over the age of 60 voted to leave the European Union.
