Maternity incidents not being reviewed - BBC News
Health boards are formally reviewing less than half of the most serious maternity incidents - which include baby deaths.
Figures obtained by the BBC showed 723 "significant adverse events" recorded across Scotland over a six-year period.
Only about 37% of these were reviewed by a full formal process, leading to concerns lessons would not be learned. [...] For example, NHS Grampian conducted an SAER in 7% of events,

[...] The merger plan had been motivated by the ability to save as much as $1bn in tax annually by shifting Pfizer’s domicile from New York to Ireland, after Allergan moved its base there a year ago in a similar move. Allergan moved its head office to Dublin after a reverse merger with the Irish drugmaker Actavis. Highest-earning Tory MP forgot to declare £400,000 income but remembered to claim 49p for milk - Mirror Online
Westminster's highest-earning Tory MP forgot to declare £400,000 worth of outside income on time - but remembered to put in expenses claims for 49p pints of milk. Backbencher Geoffrey Cox supplements his £74,000 MPs salary by performing hundreds of hours of work as a QC. He broke time limits for declaring the 500 hours of work he did between January and August last year - blaming his office clerk's retirement for the oversight. But between June and August last year, he found the time to submit four separate claims for 49p bottles of milk, all of which were refused by the expenses watchdog. His claim for £2 worth of tea bags was also knocked back.

Committee warns of decline in Scottish employment standards - BBC News
[ always announcing new jobs created #, but not the wage ] Members said there had been a "decline in job quality" in recent years, with an increase in low-paid zero-hours contract jobs since the 2008 recession. Their report said public money could be better used to support workers. The committee heard evidence from scores of organisations and took submissions from more than 600 people. Convener Murdo Fraser said the evidence from firms and workers suggested there had been a decline in job quality and an increase in low-paid jobs on "exploitative" zero-hours contracts in recent years. He said: "Whilst employment statistics point to an increase to those in work, if we look beyond the figures, we have uncovered a worrying trend in poor quality employment. [...] "We heard during our inquiry that poor quality work has a significant impact on people's health and wellbeing.

Can we really trust judges to protect reporters' confidential sources? | Media | The Guardian
Journalists won’t even be safe to talk email whistleblowers from a coffee shop [...] In stressing that the powers would not be used for mass surveillance, May said the onus would be on the home office to show necessity and proportionality. California state of emergency over methane leak - BBC News
[ saving $ on maintenance by corporate ] the well now accounts for about a quarter of the state's total emissions of methane - an extremely potent greenhouse gas. The well is situated in a mountainous area more than a mile away from residential areas, but residents have complained of health effects like headaches, nausea, vomiting and trouble breathing. "Let's call it an environmental and public health catastrophe," Tim O'Connor, a lawyer with the Environmental Defense Fund, told the BBC's environment correspondent, Matt McGrath. "In terms of timelines this is going to surpass the gulf oil problem by a mile. What we do know is that that climate equivalent of this leak is like burning thus far almost 700 million gallons of gasoline or it's the same amount of pollution as 4.5 million cars put out every day, it's tragic." [pork barrels] [...] But they boasted of using their leverage, veto threats from Obama & the GOP's craving for the oil export ban to extract numerous concessions from Republicans, including five-year extensions of solar and wind tax credits, a top goal for Reid. And they blocked numerous policy "riders" Republicans sought on the environment and other issues. "They wanted big oil so much that they gave away the store," Pelosi gloated [...] The bill extends more than 50 expiring tax cuts, with more than 20 becoming permanent, including credits for companies' expenditures for research and equipment purchases, and reductions for lower-earning families and households with children and college students. Sports Direct is a reflection of the modern British economy | Letters | Business | The Guardian
Sports Direct seems to be a metaphor for the modern British economy. “The conscious strategy would seem to be to rely on cheap labour rather than costly investment,” as your editorial (11 December) says. It also illustrates the epidemic growth of workforce insecurity which plagues a nation scarred by the rise of the precariat. Britain is rapidly becoming the sweatshop of Europe, and this, along with bubbles in consumer debt and house prices, has been a major part in such anaemic recovery as we have seen over the past five years. Becoming radicalised – and keeping a secret - BBC News
Some of these individuals identified with an organisation such as IS or al-Qaeda while others followed their own extremist beliefs and underwent a more personal form of radicalisation. [...] Still in many cases individuals are drawn to IS or other extremist groups because they're unhappy in their own lives and are looking for a cause that's bigger than themselves. Grant Shapps resigns over blackmail plot over Conservative MP's affair with an aide | Daily Mail Online
[ Tatler Tory scandal ] The Mail on Sunday wonders whether party chairman Lord Feldman - who previously shared the job with Mr Shapps - might be "doomed", after his former colleague stepped down amid claims he had failed to act on the allegations. "Shapps' allies claimed he had been made a 'scapegoat' to protect Feldman, a close friend of the prime minister," says the Mail. The Sunday ( bit.ly/1XyQr6c ) Telegraph suggests Mr Shapps must be "getting used to it", having endured controversy over claims he tried to "mould his public image on the web", his use of a pseudonym - which he initially denied - for a second job and receipt of donations from firms linked to his portfolio when a shadow minister. //via bbc.in/1OnUuBD //&! http://www.dailymail.co.uk/news/conservatives/index.html

[...] Europa hat auch ein Strukturproblem. Arbeits- und Produktmärkte sind überreguliert. Antibiotic resistance: World on cusp of 'post-antibiotic era' - BBC News
People will die, warns antibiotic expert - bbc.in/1STI2so - The world is on the cusp of a "post-antibiotic era", scientists have warned after finding bacteria resistant to drugs used when all other treatments have failed. They identified bacteria able to shrug off the drug of last resort - colistin - in patients and livestock in China. They said that resistance would spread around the world and raised the spectre of untreatable infections. It is likely resistance emerged after colistin was overused in farm animals.
