Portsmouth hospital missed lung cancer cases - BBC News
A hospital failed to spot cases of lung cancer because it did not check patients' chest X-rays properly, the Care Quality Commission has found.
The health watchdog found that three patients at Queen Alexandra Hospital in Portsmouth suffered "significant harm".
It emerged that junior doctors complained they had been asked to carry out specialist radiology work without the appropriate training.
The CQC has now launched a review of NHS radiology services in England.
All NHS bodies have been instructed to provide details about their backlogs, turnaround times, staffing and arrangements for routine reporting of images.

NHS Bosses Simon Stevens and Health Improvement have asked for 4bn more each year to meet targets again // Traditionally the health budget has got an extra 4% a year above inflation to help it cope with the ageing population and cost of new drugs and treatments.
Since 2010 that figure has dropped to close to 1% - and this extra money does little to improve that.

NHS faces even worse winter crisis than last year, watchdog warns | Society | The Guardian
In an update on how the NHS in England performed between July and September, NHSI said hospitals did not succeed in freeing up the target of 2,000-3,000 beds by September, which would have helped them cope with the extra winter demand.

NHS leaders and the health secretary, Jeremy Hunt, told hospitals in the spring that they needed to reduce the number of beds occupied due to “delayed transfers of care” – patients who are fit to leave but cannot safely be discharged – to 3.5% of their total number by September. But 5% of beds are still occupied with such cases, NHSI said.

'Radical changes' needed to £1bn stroke care - BBC News
[ 50% of disability in over 50 from Stroke ] Caring for people who have suffered from stroke is costing Wales £1bn annually, according to a leading charity.
The Stroke Association has also warned the figure could almost triple to £2.8bn by 2035 if immediate action is not taken.

NHS failing to tackle 'significant safety risks', warns its medical chief
Patients were being left at risk of suffering needless harm – including amputations, devastating maternity complications and deaths – because of the lack of action to protect them, the former heart surgeon said.

In an interview with The Telegraph, Sir Bruce said there were too many organisations vying for attention and instead, one simple system was needed to issue safety directives.

He also said too many hospitals were putting cost-cutting ahead of patient safety.

Urgent investment in trained, regulated carers is the only safe way to address this, not outsourcing the problem to random homeowners with no clearly defined standards for safeguarding, clinical governance and infection control, not to mention other risks to patients who are at their most vulnerable.

The Sunday Telegraph says trainee surgeons have complained that an endemic culture of bullying among senior colleagues is putting patients' lives at risk.
The paper says some surgeons have reported being assaulted during operations for raising safety concerns, and an atmosphere of fear is said to be leading to failures in concentration that directly harm patients.

Now they are lobbying the Government on the issue, and working with the local NHS to find alternative ways of paying for the affected services. It comes after the council had to make cuts in areas such as drug and alcohol services after the NHS withdrew £15 million last year.

Care homes places 'crisis' in five years across England - BBC News
Consumer group Which? said "urgent action" was needed because of a rising population of older people.
England has 407,000 care home beds but would need another 50,000 by 2022 to meet demand.

The row is over powers introduced in April giving NHS England the right to ration costly medicines, including if they are expected to cost more than £20m in any of their first three years of use. Theresa May’s team has adopted an ostrich-style response to the rising cash crisis and its likely impact this winter. But it’s clear that many of her MPs, fearing they could lose their reduced majorities at the next election, are pressing hard behind the scenes for a reprieve for local services.

MPs have already forced significant retreats from hospital downgrades in north Devon and Essex. Cabinet minister Andrea Leadsom has backed calls for a reprieve for hospital services at Banbury’s Horton General, and may yet have something to say about Oxfordshire’s latest bed closures.

[finding efficiencies ... ] Let’s start with costs of recruitment and retention. This is a big one. Job advertising isn’t cheap and new recruits take time to settle in. Last year, a study found there had been a 61% rise in advertising spend since 2010 in secondary schools alone, costing £56m in 2015. Shortages in the health service mean NHS Trusts are paying millions every month to agency and bank nurses to ensure there are adequate levels of staff on wards.

Stress-related leave has also increased in the public sector as workers are understandably getting run down working in understaffed schools and hospital wards. This is just staff – what about the costs to our children of being in classes too big for them to maximise their learning or to a sick individual who has an operation delayed?

Two junior doctors forced to look after 436 patients at Derriford - Plymouth Herald
The true pressures on Derriford Hospital’s junior doctors have been revealed, with serious safety concerns being raised after an incident occurred where only two staff were left to look after 436 patients during a night shift.

The issue of staff rotas and vacancies was brought up in Plymouth Hospitals NHS Trust board papers in July, in which a report outlined the feelings and concerns of junior doctors about the safety of patients.

Alzheimer’s surge adds to burden on NHS of ageing population - The Scotsman
This marks a rise of 33.4 per cent on the same period last year and more than double the 257 deaths that occurred in the same three months in 2014. A further 936 deaths in Scotland were a result of dementia, the figures showed, up 16.9 per cent over the year – although this was said in part to be due to changes in the cause of death coding software.

Growing numbers of patients are paying for private treatment to beat rationing and delays for treatment imposed by the cash-strapped NHS.

People who do not have health insurance are increasingly paying up to £14,880 for operations such as a hip or knee replacement or cataract removal, a report reveals.

Profit-driven hospital firms are experiencing 15 to 25% year-on-year rises in the number of uninsured “self-payers”, with the increase mainly driven by long waiting times to undergo non-urgent surgery in NHS hospitals. Patients are using their savings or taking out loans to pay for their treatment. "The vast majority of dentists want to support the NHS, but we're not miracle workers and a bankrupt dentist is no good to anyone," said Dr Tony Kilcoyne, who runs a private dental practice in Keighley, West Yorkshire.
"The vast majority of dentists are self-employed so the government doesn't pay our staff or our overheads like a hospital.
"If the system is funded at about half the level that it needs to be, then we can't treat everyone." [...] "The government has only commissioned enough dentistry to treat about half the adult population and this is an absolute disgrace."
