Hospital bed cutbacks have gone too far, NHS England boss says | Society | The Guardian
Policy has left hospitals unable to cope with rise in patient numbers, Simon Stevens says [...] The number of beds in NHS hospitals and other facilities has fallen from 144,455 in April to June 2010 to 129,992 in January to March 2018 – a cut of 14,463 or 10% of the total.

Stroke deaths in England halved in ten years, study finds | NIHR Oxford Biomedical Research Centre
global burden of disease

The rate of people dying from strokes decreased by 55% in a decade, according to a study published in the BMJ (British Medical Journal).

Researchers from the University of Oxford analysed data from almost 800,000 adults in England admitted to hospital between 2001 and 2010.

The authors said: "Our findings show that most of the reduction in stroke mortality is a result of improved survival of patients with stroke.

"However, acute and long-term management of such patients is expensive, and the NHS is already spending about 5% of its budget on stroke care.

"By focusing on prevention and reducing the occurrence of stroke, major resources can be conserved."

The study found that death rates from strokes decreased across all age groups. In men, overall death rates dropped from 140 per 100,000 people in 2001, to 74 per 100,000 people in 2010, and in women, from 128 per 100,000 to 72 per 100,000.

Inequality driving 'deaths of despair' - BBC News
there is income inequality
there is health inequality (air)
there is political inequality, your voice only heard when you have money to donate and can pay lobbyists.
>> electoral reform (1), limit lobby influence and party donations (2), citizens assembly, devolution might have failed.

Widening inequalities in pay, health and opportunities in the UK are undermining trust in democracy, says an Institute for Fiscal Studies report. [...] The think tank warns of runaway incomes for high earners but rises in "deaths of despair", such as from addiction and suicide, among the poorest.

It warns of risks to "centre-ground" politics from stagnating pay and divides in health and education.

The report says such widening gaps are "making a mockery of democracy".

The Institute for Fiscal Studies (IFS), one of the country's leading research institutes, is launching what it says is the UK's biggest analysis of inequality.

That will be chaired by Nobel Prize-winning economist Prof Sir Angus Deaton.

400 care home operators collapse in five years as cuts bite | Society | The Guardian
BDO’s research said care homes, already struggling with debts racked up before the credit crunch, had “suffered as a result of the reduced spending by central government”.

It warned that further pain lay ahead, highlighting research from the Association of Directors of Adult Social Services showing that councils had £700m of social care cuts planned in 2018-19, despite growing demand.

Alexandria Ocasio-Cortez, Crusher of Sacred Cows – Rolling Stone
With its silly swipes at AOC, the American political establishment is once again revealing its blindness to its own unpopularity [...] The mortification on the Republican side has come more from media figures than actual elected officials. Still, there are plenty of people like Gov. Ron DeSantis (R-FL) doing things like denouncing “this girl, Alexandria Ocasio-Cortez, whatever she is” for preaching “socialism wrapped in ignorance.” A group of GOP House members booed her on the floor, to which she replied, “Don’t hate me cause you ain’t me, fellas.” [...] The latter issue, of course, is the real problem most of Washington has with “AOC”: her self-generated popularity and large social media presence means she doesn’t need to ask anyone’s permission to say anything. [...] She doesn’t have to run things by donors and she doesn’t need the assent of thinkfluencers like Cillizza or Max Boot (who similarly compared her to both Trump and Sarah Palin), because she almost certainly gains popularity every time one of those nitwits takes a swipe at her.

At the heart of this NHS cash boost lies a dishonesty: tax | Rafael Behr | Opinion | The Guardian
Theresa May’s boast that extra funding won’t involve tax rises should set alarm bells ringing [...] Stevens knows that £20.5bn is a lot, yet not enough. It repairs some of the damage inflicted by austerity – a problem denied for years by ministers who insisted the NHS budget was ring-fenced. That was true in a narrow technical sense. But rising costs and a displaced burden from deep cuts elsewhere imposed an unbearable strain on hospitals and GPs. [...] Phased in over five years, the £20.5bn uplift temporarily restores health spending to something like its pre-austerity trajectory. But it is unclear where the next £20.5bn comes from. Or the £20.5bn after that. The Institute for Fiscal Studies estimates that budgets should rise by 3.3% over 15 years to keep services at their current level. If the ambition is to make them better, the annual increase would be more like 5%, taking health spending to around 10% of national income. That is the level maintained by countries like the Netherlands and Sweden. May has no plan to keep Britain in that league. [...] The dishonest part is making it sound like a self-sustaining process when it is a one-off event.

New leadership, same old problems - BBC News
new money won't be seen till next year, new winter crisis is coming for sure, and the wait lists will get even longer. - Health think tanks argue that the 3.4% is really only enough to cope with rising demand, never mind improving care.

Hospital sources point out that the new money won't come to the NHS until next year.

Nothing extra so far has been allocated for this winter and the intense pressure seen in the first months of this year is likely to be repeated.

There is every chance that waiting lists for routine surgery will get longer and accident and emergency performance won't get better.

Even with the £6bn cash boost next year, there are warnings that there won't be any rapid improvement. His instincts will be to tell the service to get on with job with the money it has. Theresa May faces dilemma over NHS cash boost, says IFS | Business | The Guardian
PM will have to either raise taxes, slash public spending or break her own spending rules - “The government is in a bind,” the IFS said. “It is extremely doubtful that large additional cuts to spending on other public services are either feasible politically or consistent with maintaining quality.

“So unless it is able and willing to implement tax rises or further cuts to the social security budget over the rest of this parliament it is hard to see how a significant injection of additional cash into the NHS would be consistent with the government’s stated fiscal objective.”

Bev Humphrey: ‘The underfunding of the NHS is almost conspiratorial’ | Society | The Guardian
Humphrey believes the health service is now close to breaking point and that government policy is to blame. “The chronic underfunding of the NHS is almost conspiratorial,” she says. “The way it has been starved of money for the last seven years is scandalous. We haven’t got to this decade to find everyone is suddenly iller. I fear the government will turn around in a few years and say, ‘Look, we have given you money and it hasn’t worked – we need a new system.’ I am concerned this would lead to a compulsory insurance model.” It would create a two-tier system, she feels. “The NHS is not only a very efficient service, it’s also clinically excellent. If we move away from that [a service that is free at the point of need] there’s no going back. What you would end up with is very poor quality services for the poorest and those who need it most.”

The Daily Mail says strict rationing rules being used by three quarters of NHS hospitals means patients in need of a replacement knee or hip must have to use a walking stick or wheelchair before they will be considered for surgery.

It adds that patients who are told they can't have an operation are advised to install hand rails at home, perform stretches or buy special shoes.

NHS workforce 'at crunch point' - BBC News
The UK's medical profession is at a "crunch point", facing the prospect of too few doctors to treat rising numbers of patients, the regulator says.

The General Medical Council says the supply of medics has failed to keep up with demand and warns against the over-reliance on overseas staff post-Brexit.

The GMC's Charlie Massey called it a "crucial moment" for UK healthcare.

It comes despite government promises in England to increase the number of doctors in training.

NHS Grampian puts cap on non-urgent surgery over winter - Evening Express
NHS Grampian has confirmed it is only booking category one and two patients, who require surgery within a month or within three months respectively, in January and February.

The health board said the move was part of its winter plans and there would be exceptions where surgeries do not impact on beds. [...] It follows a row in October after NHS Tayside announced it had put on hold all planned, non-urgent operations for three weeks over the festive period. Category three patients, who require surgery within a year, will not be routinely booked for January and February, under NHS Grampian’s plans.

Jeremy Hunt orders NHS England bosses to stick to waiting time limits
Health secretary swiftly rejects service chiefs’ public warning that meeting targets is impossible with current funding [...] In an unprecedented move reflecting a widening gulf with the government, NHS England used its board meeting to warn that it had to make “difficult choices” next year that would involve limiting what it could provide for patients. [ only providing basics life saving treatments - not life improvement treatments (elective surgery) ] [...] NHS England has also said it is unable to implement best practice as advised by Nice on a routine basis – an extraordinary state of affairs. We have now reached a point where the NHS constitution will be routinely breached and NHS services are being withdrawn.”

Budget 2017: NHS trusts given extra £350m for entire winter, despite Brexit bus promising £350m a week | The Independent
Budget had also nothing on social care! - &! begging patients &! more rationing and restrictions in the books by CCGs eg physical rehabilitation services could be cut. &! // The NHS is getting £1.6bn in extra funding next year, less than half asked for by the NHS to keep the service running //&! - GP leaders hit out at chancellor Philip Hammond for ignoring general practice in his 2017 budget, as NHS England officials hinted that a 'disappointing' £2.8bn funding hike for the health service could force cuts. //&! - the NHS “can no longer avoid the difficult debate” on what can be provided by the health service on the funds it is operating on. //&! - An analysis carried out for Labour by the House of Commons library shows that despite the chancellor, Philip Hammond, announcing an extra £2.8bn in day-to-day funding, real-terms funding per head is set to fall in 2018-19, and remain flat for two years after that.

NHS Budget plan not enough, say bosses - BBC News
NHS Bosses Simon Stevens and Health Improvement have asked for 4bn more each year to meet targets again // Traditionally the health budget has got an extra 4% a year above inflation to help it cope with the ageing population and cost of new drugs and treatments.
Since 2010 that figure has dropped to close to 1% - and this extra money does little to improve that.
Those working in accident and emergency units point out that the £350m for this winter could easily be swallowed up by spending on agency staff.
It is why staff on the front line are still predicting rising waiting times for treatment.

Jeremy Hunt's GP recruitment pledge in tatters as 1,000 full-time NHS doctors quit last year | The Independent
Despite £20,000 incentives to attract trainees and attempts to hold on to senior staff, the NHS is now 1,200 GPs further away from its target of recruiting an extra 5,000 GPs by 2020 than when it was first set by Health Secretary Jeremy Hunt in September 2015.

The Department of Health baseline at that time was 34,592 full-time GPs, but two years on the September 2017 figures show overall numbers stand at 33,302. &!

Finance trumps patients at every level – UK healthcare needs an inquiry | Aseem Malhotra | Healthcare Professionals Network | The Guardian
tl;dr Diet & Lifestyle changes of the nation in the short-, medium- and long-term term can help (substantially) save the NHS. // A full public inquiry on the scale of Chilcot into the Iraq war may ultimately be required to overhaul a system where at every level financial interest continues to trump what’s best for patients – not only to improve the quality of care, but to restore trust in medical research. Until then, NHS England, the BMA and the Department of Health must make it a national priority to emphasise to patients that less medicine and healthier lifestyles are the most important solutions to sustaining the NHS. // statistics use in medical research and patient selection and reporting issue. absolute risk v % of all patients. // Such statistical and health illiteracy, far from being the exception, is actually the norm. The majority of healthcare professionals seem unable to understand medical literature. One study revealed 70% of those on a training programme failed a simple test on their understanding of evidence-based medicine.

NHS faces even worse winter crisis than last year, watchdog warns | Society | The Guardian
In an update on how the NHS in England performed between July and September, NHSI said hospitals did not succeed in freeing up the target of 2,000-3,000 beds by September, which would have helped them cope with the extra winter demand.

NHS leaders and the health secretary, Jeremy Hunt, told hospitals in the spring that they needed to reduce the number of beds occupied due to “delayed transfers of care” – patients who are fit to leave but cannot safely be discharged – to 3.5% of their total number by September. But 5% of beds are still occupied with such cases, NHSI said.
