Funding the NHS: Is it finally time for a new approach? - BBC News
A ring-fenced tax for health and social care has been resisted in the past by the Treasury. Such a policy is known by economists as hypothecation. Opponents, including Treasury mandarins, believe it ties the hands of chancellors.

But, intriguingly, a former top civil servant at the Treasury, Sir Nick Macpherson, has come out in favour of a ring-fenced tax which is renewed every five years.

Cold Comfort: So-Called ‘Tough Decisions’ Are Putting Patients at Risk | Novara Media
As shown by the paradise papers, many PFI contracts are held in offshore trusts. So we are all being doubly ripped off, tied into open-ended blank chequebooks, with extortionate amounts being paid by near-bankrupt health authorities to companies that pay no UK tax. The health authorities are unable to employ locum staff to support the unfortunates working on the wards, meaning everyone is overstretched. No wonder austerity has likely resulted in 45,000 extra deaths in the NHS since austerity was introduced in 2010, as the BMJ Open study estimates.

NHS workforce 'at crunch point' - BBC News
The UK's medical profession is at a "crunch point", facing the prospect of too few doctors to treat rising numbers of patients, the regulator says.

The General Medical Council says the supply of medics has failed to keep up with demand and warns against the over-reliance on overseas staff post-Brexit.

The GMC's Charlie Massey called it a "crucial moment" for UK healthcare.

It comes despite government promises in England to increase the number of doctors in training.

NHS Grampian puts cap on non-urgent surgery over winter - Evening Express
NHS Grampian has confirmed it is only booking category one and two patients, who require surgery within a month or within three months respectively, in January and February.

The health board said the move was part of its winter plans and there would be exceptions where surgeries do not impact on beds. [...] It follows a row in October after NHS Tayside announced it had put on hold all planned, non-urgent operations for three weeks over the festive period. Category three patients, who require surgery within a year, will not be routinely booked for January and February, under NHS Grampian’s plans.

NHS Budget plan not enough, say bosses - BBC News
NHS Bosses Simon Stevens and Health Improvement have asked for 4bn more each year to meet targets again // Traditionally the health budget has got an extra 4% a year above inflation to help it cope with the ageing population and cost of new drugs and treatments.
Since 2010 that figure has dropped to close to 1% - and this extra money does little to improve that.
Those working in accident and emergency units point out that the £350m for this winter could easily be swallowed up by spending on agency staff.

Finance trumps patients at every level – UK healthcare needs an inquiry | Aseem Malhotra | Healthcare Professionals Network | The Guardian
tl;dr Diet & Lifestyle changes of the nation in the short-, medium- and long-term term can help (substantially) save the NHS. // A full public inquiry on the scale of Chilcot into the Iraq war may ultimately be required to overhaul a system where at every level financial interest continues to trump what’s best for patients – not only to improve the quality of care, but to restore trust in medical research. Until then, NHS England, the BMA and the Department of Health must make it a national priority to emphasise to patients that less medicine and healthier lifestyles are the most important solutions to sustaining the NHS. // statistics use in medical research and patient selection and reporting issue. absolute risk v % of all patients. // Such statistical and health illiteracy, far from being the exception, is actually the norm. The majority of healthcare professionals seem unable to understand medical literature. NHS faces even worse winter crisis than last year, watchdog warns | Society | The Guardian
In an update on how the NHS in England performed between July and September, NHSI said hospitals did not succeed in freeing up the target of 2,000-3,000 beds by September, which would have helped them cope with the extra winter demand.

NHS leaders and the health secretary, Jeremy Hunt, told hospitals in the spring that they needed to reduce the number of beds occupied due to “delayed transfers of care” – patients who are fit to leave but cannot safely be discharged – to 3.5% of their total number by September. But 5% of beds are still occupied with such cases, NHSI said.

Thanks but no thanks, CareRooms. In the NHS we need staff, not beds | Rachel Clarke | Opinion | The Guardian
Trained staff – carers – are what the sector lacks, not physical spaces in buildings. Urgent investment in trained, regulated carers is the only safe way to address this, not outsourcing the problem to random homeowners with no clearly defined standards for safeguarding, clinical governance and infection control, not to mention other risks to patients who are at their most vulnerable.

NHS suspends operations over Christmas and new year - BBC News
NHS Tayside has put on hold all planned, non-urgent operations for three weeks over the festive period.
The health board said emergency surgery, urgent procedures and all cancer surgeries would still be carried out.
But there will be a "step-down of all planned procedures" at Perth Royal Infirmary, Ninewells Hospital in Dundee and Stracathro Hospital, near Brechin.
The disruption will last from 18 December to 8 January 2018. & https://www.eveningtelegraph.co.uk/fp/nhs-tayside-chairman-predicts-serious-difficulties-current-level-spending-continues/ - Tayside to save 200m over 5 years ... &! http://www.heraldscotland.com/news/15626138.Crisis_at_NHS_Tayside_deepens_over___6m_overspend/ closed 60 beds

Doctors to breathalyse smokers before allowing them NHS surgery | Society | The Guardian
He said that by introducing “increasingly harsher policies” CCGs might believe they were alleviating financial pressure but they could actually cost more in the long term because of the risk of complications and the cost of healthcare in the interim. The CCGs said no financial savings were expected as a result of the measures, which they claimed were aimed at improving the long-term health of residents. They also stressed that if at any time the harm of waiting for surgery outweighed the benefit of losing weight or stopping smoking, then the patient would be referred.

Jeremy Hunt considers barring walk-in patients from A&E | Society | The Guardian
“We know that the main issue contributing to pressures in A&E is not inappropriate attendance, but a lack of hospital beds, and a lack of the community support services patients need when they are discharged, so they end up staying in hospital for longer than they needed to.” [...] She added: “Maybe a pilot will identify some good ways of reducing genuinely inappropriate use of A&E, but that this idea is even circulating speaks volumes about the consequences of the government’s decision to underfund the NHS. The chancellor must take note, and respond in his budget next month.”

NHS operations could be cancelled for months this winter, senior medics warn
"If, as we expect, we see a flu outbreak, I think the NHS - in terms of both beds and clinical staff - will break far worse than last year and elective activity will almost certainly cease for several months." [...] Meanwhile, leaked documents seen by Health Service Journal suggest that an NHS trust with a history of high death rates agreed savings plans, despite warnings from its then finance director that doing so could threaten patient safety.

Falling number of nurses in the NHS paints a worrying picture | The King's Fund
Similar recruitment and retention problems have arisen with GPs, but here, interestingly, the government is intending to pull the international recruitment lever with a vengeance. Time will tell how successful this will be in the current environment: the plan is to attract at least some of these from other EU countries and as the nursing numbers show us, the UK is no longer quite so attractive a destination for clinical staff for obvious reasons.

At a deeper level, Brexit has again exposed the fragility of NHS workforce planning. This may be most immediate for nursing but the need for an integrated short and long-term plan for the NHS workforce long precedes Brexit. After all, there will be little point in giving the NHS its (disputed) extra £350 million a week if there are no staff to spend it on.

A Simple Way to Involve Frontline Clinicians in Managing Costs
NHS hospitals exploring how to tighten the belt // This testing in NHS Scotland has taught us several lessons about the approach to managing value in a busy health care environment. First, point-of-care leaders, if armed with information about costs and quality, can improve both. // debate abt value v overtreatment. nurses and doctors now not only have to navigate overtreatment, patient safety/satisfaction but also keep in mind the purse of the nation v every single one patient they are seeing.

Council and NHS in £19.5m funding row over patients staying in hospital too long - Stoke Sentinel
[first NHS Councils blame each other for little progress, now NHS Department withholding funds bc no progress from Council ] But now NHS England is threatening to withhold the cash (one tranche from the Better Care Fund) because the authority is failing to reduce its bed-blocking figures by ensuring care packages are in place. Council leaders say the target – cutting social care-related delayed transfers of care by two-thirds in eight weeks – was unrealistic and set at ‘extremely short notice’. Now they are lobbying the Government on the issue, and working with the local NHS to find alternative ways of paying for the affected services. It comes after the council had to make cuts in areas such as drug and alcohol services after the NHS withdrew £15 million last year.

NHS future precarious, says regulator - BBC News
The Care Quality Commission (CQC) says the health system is "straining at the seams" and faces a "precarious" future.

It said so far the quality of NHS and council care has been maintained but warned standards were likely to drop. //>> GPs not focused to reverse diet lifestyle diesease. // "We are going to see a fall in the quality of services that are offered to people and that may mean that the safety of some people is compromised,"

At last, we seem to be awakening from the bad dream of “tick-box medicine”
Doctors allowed themselves to be reduced to automatons. [...] NICE has recently changed the rubric that prefaces its guidelines, stressing that they are not mandatory and that they must be tailored to each individual patient. This would once simply have been good practice. That it has become necessary for NICE to spell it out shows just how far the profession has been degraded by the tick-box culture. [...] reduced to automatons, and their patients to pill-swallowing machines.
