Health boards are formally reviewing less than half of the most serious maternity incidents - which include baby deaths.
Figures obtained by the BBC showed 723 "significant adverse events" recorded across Scotland over a six-year period.
Only about 37% of these were reviewed by a full formal process, leading to concerns lessons would not be learned. [...] For example, NHS Grampian conducted an SAER in 7% of events,

Redaction error reveals FBI did target Lavabit to spy on Edward Snowden
A redaction oversight by the US government has finally confirmed that the Federal Bureau of Investigation’s targeting of secure email service Lavabit was used specifically to spy on Edward Snowden.

Ladar Levison, creator of the email service, which was founded on a basis of private communications secured by encryption and had 410,000 users, was served a sealed order in 2013 forcing him to aid the FBI in its surveillance of Snowden.

Levison was ordered to install a surveillance package on his company’s servers and later to turn over Lavabit’s encryption keys so that it would give the FBI the ability to read the most secure messages that the company offered. He was also ordered not to disclose the fact to third-parties.

Draft snooper's charter 'fails on spying powers and privacy protections' | World news | The Guardian
Theresa May’s draft “snooper’s charter” bill fails to cover all the intrusive spying powers of the security agencies and lacks clarity in its privacy protections, a parliamentary committee has said. The intelligence and security committee said the draft legislation published by the home secretary suffered from a lack of sufficient time and preparation. It was evident that even those working on the legislation had not always been clear about what it was intended to achieve, it said.

Can we really trust judges to protect reporters' confidential sources? | Media | The Guardian
Journalists won’t even be safe to talk email whistleblowers from a coffee shop [...] In stressing that the powers would not be used for mass surveillance, May said the onus would be on the home office to show necessity and proportionality. They would operate a “double-lock” authorisation process on intercept and bulk interception warrants in order to prevent intrusive snooping. Sports Direct is a reflection of the modern British economy | Letters | Business | The Guardian
Sports Direct seems to be a metaphor for the modern British economy. “The conscious strategy would seem to be to rely on cheap labour rather than costly investment,” as your editorial (11 December) says. It also illustrates the epidemic growth of workforce insecurity which plagues a nation scarred by the rise of the precariat. Britain is rapidly becoming the sweatshop of Europe, and this, along with bubbles in consumer debt and house prices, has been a major part in such anaemic recovery as we have seen over the past five years. How a whistleblower brought down Chicago police chief - BBC News
It was a common story in Chicago. According to the Better Government Association, Chicago police shot 240 people over a four-year span, or about one per week. Seventy of those people died and of those, two-thirds were African American. The city's Independent Police Review Authority has investigated 400 police shootings since 2007 and found only one incident to be unjustified. It seemed as though McDonald's case would be chalked up as another justified use of deadly force. But the person on the phone told Futterman a very different story. He said the shooting was captured on a police vehicle's dash camera.

NHS staff complaints rise (From Herald Scotland)
WORKERS at a crisis-hit health board have raised hundreds of concerns about staffing issues over the past two years.  New figures released by NHS Grampian show that 781 employees complained to management about staff shortages at hospitals across the north-east of Scotland in 2014-15. This compared to 625 complaints about recruitment the previous year. The health board is in charge of several hospitals, including Woodend Hospital in Aberdeen and Dr Gray’s Hospital, Elgin. Aberdeen Royal Infirmary is NHS Grampian’s largest hospital and has been at the centre of a staffing crisis over the past two years. // &! Health Boards best practice when Doctors complain? Refer them to the medical council, making their lives difficult: A HEALTH board at the centre of a row over the way they treat whistleblowers has referred more doctors to the General Medical Council in the last 17 months than the largest health authority in the UK. -bit.ly/1Pij32T //&! bit.ly/1JSs8bp spending on locum staff

Russia will fork Sailfish OS to shut out pesky Western spooks
Paranoia over NSA tampering spurs de-Westernisation drive // well only the Paranoid survive. [...] Russia’s Minister of Communications and Mass Media, Nikolai Nikiforov, has taken part in talks to form a consortium that will aid Russia in developing a custom mobile OS, reportedly a forked version of Jolla’s Sailfish OS, to lessen its dependence on Western technology. Nikiforov held a working meeting last week with the leadership of Hong Kong-based Sailfish Holdings, developer of the quietly admired Sailfish OS. Alongside other participants, the parties involved discussed the use of open-source software in the public and private sectors, taking into account technical, economic and political factors, the latter of which is expected to include United States’ interference with exported technologies.

Is this the end for the Freedom of Information Act? - BBC News
The commission's remit is to look at two substantive issues that officials keep complaining about: "whether the operation of the Act adequately recognises the need for a 'safe space' for policy development and implementation and frank advice", and whether complying with the act is too much of a burden. [...] The safe space? So what about the "safe space" arguments? Under current law, section 35 of the Act allows the withholding of policy development work, private office communications & letters between ministers. If that isn't enough, section 36 allows ministers to withhold other information that would undermine the safe space for discussion. [...] That is, more or less, the information that a good transparency law should publish. It is supposed to help us find cover-ups, incompetence and crookedness by the powerful. [...] there have been no individual releases that have actually caused any problems. [well, if u dont know what 2 ask 4 specifically, u dont ask #insider #whistleblower ]

Kaspersky claims US spies can infect computers with malware that is almost impossible to detect | PandoDaily

A court managed what the complicit UK press couldn't: force GCHQ to tell the truth | Trevor Timm | Comment is free | The Guardian
A UK court vindicated Edward Snowden’s whistleblowing on Friday by ruling that the secrecy surrounding one of the programs he exposed was, in fact, illegal. The decision is more evidence that not only were the Snowden revelations necessary and justified, but are also slowly forcing changes in both US and UK, even as both governments fiercely resist. In a stunning ruling, the Investigatory Powers Tribunal (IPT) – which oversees (and usually rubber stamps) Britain’s spy agencies – declared that the intelligence-sharing rules between the NSA and GCHQ (Britain’s NSA equivalent and partner-in-crime) governing their mass surveillance program violated UK human rights laws because they were kept secret for so long. [...] Snowden once rightly called the UK media’s coverage of the GCHQ story “a disservice to the public.”

Here’s how reporters are reacting to Snowden’s revelations | PandoDaily
The global surveillance programs revealed by Edward Snowden have caused journalists to change how they operate, question what the government knows about them, and consider abandoning investigative reporting, according to a survey from the Pew Research Center. A significant number of the journalists surveyed said they had changed their behavior, whether it’s how they store sensitive files (49 percent), how they communicate with other journalists (29 percent), whether they’ve reached out to a source (13 percent), or pursued a specific story (2 percent). Many are also convinced the government is gathering information about them. Some 64 percent of journalists believed intelligence agencies had “probably collected data” about them. (The other 36 percent, as “InfoSec Taylor Swift” joked, must not read the news.)

Today's Uncertain Future Calls For Observation and Patience, Not Boldness and Strategy | BillMoyers.com
Not one of the signature foreign policy initiatives conceived in Obama’s first term has borne fruit. When it came to making a fresh start with the Islamic world, responsibly ending the “dumb” war in Iraq (while winning the “necessary” one in Afghanistan), “resetting” US-Russian relations, and “pivoting” toward Asia, mark your scorecard zero for four. There’s no doubt that when Kerry arrived at the State Department he brought with him some much-needed energy. That he is giving it his all — the department’s website reports that the secretary has already clocked over 682,000 miles of travel — is doubtless true as well. The problem is the absence of results. Remember when his signature initiative was going to be an Israeli-Palestinian peace deal? Sadly, that quixotic plan, too, has come to naught.

Beds crisis hits NHS care for mentally ill children | Society | The Guardian
Emails reveal emergency new steps as teenagers aged 16-18 are put on adult wards rather than in specialist adolescent facilities [...] Labour’s shadow minister for public health, Luciana Berger, described the situation as “utterly appalling” and blamed the crisis on £50m of cuts to children’s mental health services since 2010. [...] Pressure on the government over the NHS mounted further as Stephen Dorrell, the former Tory health secretary, says in an Observer interview that reforms of the health system pushed through by Andrew Lansley in 2012 did not address the real problem: the need to keep people out of hospital by improving care services. Dorrell said the reforms were the worst mistake of this government and accused ministers of being more interested in pursuing headlines than a policy which addressed the NHS’s real needs.

Apple may have compromised everyone’s security for access to China | PandoDaily
But if Apple really did provide access to the iPhone operating system’s source code, it should admit as much so people know the degree to which their security may be compromised, all for the sake of Apple’s business ambitions.

Canadian intel program monitored global file transfers to find potential extremists | PandoDaily
LEVITATION is said to work by collecting information from Internet cables to which the Canadian equivalent to the National Security Agency has access. It then looks at the affected IP addresses and uses them to cross-reference its findings with other databases. It is said to monitor between 10 million and 15 million uploads and downloads within certain countries inside North Africa, Europe, the Middle East, and North America. Most of those transfers are monitored in vain: the program is said to reveal just 350 “interesting” downloads each month — less than 0.0001 of the digital traffic it surveils. Indeed, a member of the Canadian intelligence agency is said to have joked about the number of “Glee” downloads it has to sift through to find anything remotely relevant.

BADASS intelligence program shows there’s no distinction between for-profit and governmental surveillance | PandoDaily
Put another way: so long as government agencies are able to access or intercept data collected by private services, there is no distinction between for-profit surveillance and government spying programs. All the information eventually goes to the same place.

Google did fight to reveal warrant for WikiLeaks journalists’ Gmail accounts, attorney says | PandoDaily
bit.ly/1EnsklH - [any american company, or company with american satellite office is not deemed safe. that that is just the tip of the iceberg.] An attorney for Google has told the Washington Post that the company did fight a gag order preventing it from telling three WikiLeaks journalists their Gmail accounts were monitored by the Justice Department as the result of a warrant issued in March 2012. The statement was made in response to questions from WikiLeaks regarding Google’s compliance with the warrant, the existence of which it didn’t reveal to its subjects until December 2014, more than two-and-a-half years after the warrant was first issued. The warrant allowed the FBI to receive metadata and the full content of any messages sent via the monitored accounts. (WikiLeaks says the Gmail accounts were for personal use and that all sensitive conversations take place via encrypted communications tools.)
