Portsmouth hospital missed lung cancer cases - BBC News
A hospital failed to spot cases of lung cancer because it did not check patients' chest X-rays properly, the Care Quality Commission has found.
The health watchdog found that three patients at Queen Alexandra Hospital in Portsmouth suffered "significant harm".
It emerged that junior doctors complained they had been asked to carry out specialist radiology work without the appropriate training.
The CQC has now launched a review of NHS radiology services in England.
All NHS bodies have been instructed to provide details about their backlogs, turnaround times, staffing and arrangements for routine reporting of images.

NHS Bosses Simon Stevens and Health Improvement have asked for 4bn more each year to meet targets again // Traditionally the health budget has got an extra 4% a year above inflation to help it cope with the ageing population and cost of new drugs and treatments.
Since 2010 that figure has dropped to close to 1% - and this extra money does little to improve that.
Those working in accident and emergency units point out that the £350m for this winter could easily be swallowed up by spending on agency staff.
It is why staff on the front line are still predicting rising waiting times for treatment.

But as common as antidepressants are, the fact remains the majority of people with mental illness report they are not getting help.

[finding efficiencies ... ] Let’s start with costs of recruitment and retention. This is a big one. Job advertising isn’t cheap and new recruits take time to settle in. Last year, a study found there had been a 61% rise in advertising spend since 2010 in secondary schools alone, costing £56m in 2015. Shortages in the health service mean NHS Trusts are paying millions every month to agency and bank nurses to ensure there are adequate levels of staff on wards.

Stress-related leave has also increased in the public sector as workers are understandably getting run down working in understaffed schools and hospital wards. This is just staff – what about the costs to our children of being in classes too big for them to maximise their learning or to a sick individual who has an operation delayed?

Mergers of GPs and CCGs and Super Mergers like rumors in Scotland for 4-5 super trusts eg one NE Trust is the new internal thinking to cope with the political challenge of political austerity. STPs and rationing (prioritising cancer and emergency care) just the microcosm or finding £ efficencies but not with the aim of improving patient safety, satisfaction and outcomes. And also making things unequal - preferred treatment decisions taken based on what ill you have and how ill you are and if it is life threatening.

It points out the obsession with market forces is undermining the development of new ways of delivering services, and highlights the obvious but little discussed fact that STPs do not legally exist, so they have no authority to implement the changes they are recommending. They have been stitched together to overcome the structural chaos ushered in by Andrew Lansley. [...] Labour has to build a credible response to these problems. The Copeland defeat shows “save our NHS” will not be enough to save the Labour party. If it is going to demonstrate it is ready for government it will have to stop writing its health policy on a placard.

[ NHS is political football ] Healthcare Improvement Scotland (HIS) was launched in 2011 to improve care standards, although it has few statutory enforcement powers.

As it is part of the NHS, campaigners, politicians and even the OECD world forum have said the health service is effectively “marking its own homework”. HIS has inspected dozens of hospitals and health boards over the past six years, exposing many care, staffing and cleanliness problems. [...] She said: “There is a classic example from NHS Grampian, when there were major issues… We triangulated information with the General Medical Council and the consultants, and we also met patient groups in Grampian. During that time and before the report was published, the report was shared with Government, not so that changes could be made but so that action could be taken on some of the serious problems in Grampian.”

It noted that pressures on mental health services were particularly acute, with a 44% decrease in the number of mental health beds since 2000-01. [...] “Ultimately we could reduce the need for hospital beds by improving preventive care. But cutting both preventive services and beds leads to disaster. That is what we are now witnessing.”

Frail, elderly people 'left to struggle alone' - BBC News
Hundreds of thousands of vulnerable older people struggle to get by with little or no care because of cuts to care in England, a charity says. [...] And the charity warned that the whole system was close to collapse in some areas with services closing.
It comes after the government said it was looking to solve the problems.
Last week, Health Secretary Jeremy Hunt pledged action on social care, which covers care homes and help in the home.
That has prompted a growing expectation that a rescue package will be announced in the Budget in early March.
