Doctors warn of alarming C-section ′epidemic′

Elizabeth Warren building unlikely connection with black female voters
HOUSTON >> Elizabeth Warren was the last of eight presidential candidates to take the stage at Texas Southern University last month when she was pressed for a solution to black women dying during childbirth at far higher rates than white women.

The Massachusetts senator responded with what has become a campaign catchphrase: “So, I got a plan.” She proposed holding hospitals financially responsible for the disparity, imposing penalties on institutions that don’t act to prevent such deaths.

“Doctors and nurses don’t hear African American women’s medical issues the same way that they hear the same things from white women,” she said. “We’ve got to change that, and we’ve got to do it fast because people’s lives are at stake.”

By the time Warren left the stage at the “She the People” forum, thousands of black women in the audience were on their feet roaring cheers and applauding. The reaction eclipsed the response earlier in the day to Sens. Kamala Harris of California and Cory Booker of New Jersey — the black candidates in the Democratic contest. It reflected the unlikely traction that Warren, a 69-year-old white woman who lives in tony Cambridge, Massachusetts, is gaining with black women who are debating whom to back in a historically diverse primary.

“To have an ally — she’s a woman, but she’s not a black woman — who can speak intelligently and has thought about people who don’t look like you, that resonates,” said Roxy D. Hall Williamson, a 49-year-old who was in the audience. “She stole the show to me.”

China Wants More Anesthesiologists, Vaginal Births
According to a 2010 study by the World Health Organization, c-sections were more common in China than in eight other Asian counties surveyed. A 2017 study found that c-sections accounted for 29 percent of China’s births in 2008 but that this figure had risen to 35 percent by 2014. According to the WHO, the ideal range for c-sections is 10–15 percent.

Given the health risks of c-sections — including infections, blood clots, and complications during future pregnancies — the Chinese government is intent on making them a secondary option rather than the default. But as long as it has so few qualified professionals, it’s going to have a hard time selling this plan. According to statistics from the World Federation of Societies of Anaesthesiologists, China has just 5.12 physician anesthesia providers per 100,000 people.

Furthermore, a 2014 survey of nearly 13,000 anesthesiologists in China found that 57 percent said they would opt not to pursue the profession if given another chance, and 64 percent said they worked over 50 hours per week.

“Anesthesiologists have to be on call 24 hours a day. If they’re on the night shift, it’s rare for them to get to sleep in a bed,” said Fu. “Whenever there’s an expectant mother who requires anesthesia during labor, the obstetrician will give us a call.”

In August 2018, several top government organs including the National Health Commission and the Ministry of Education published a joint guideline aiming to increase the number of anesthesiologists in China to 90,000 by 2020, 140,000 by 2030, and 160,000 by 2035. According to the World Federation of Societies of Anaesthesiologists, the country currently has around 71,700 physician anesthesia providers.

High pressure and long working hours have become an added deterrent to anyone considering entering the profession. In March 2015, a 26-year-old anesthesiologist at a hospital in the eastern Zhejiang province died unexpectedly, purportedly from overwork. And in October 2014, an anesthesiologist in his 40s fell into a coma after suffering a stroke in the operating room. In its story about the 2014 incident, China National Radio reported that around 15 anesthesiologists in China had died suddenly over the previous two years.

The root cause of these startling statistics is often misunderstood. The public image of maternal death is a woman who has a medical emergency like a hemorrhage while in labor. However, very few deaths counted in maternal mortality statistics occur during childbirth. Rather, four out of five of these deaths happen in the weeks and months before or after birth. So, they occur not in the hospital, but in our communities. And they represent many failures — not just unsafe medical care, but also eroding social support necessary for women to recognize medical warning signs, like abnormal bleeding or hopelessness about the future, and to seek timely care.

A few days after having a baby, American women are sent home from the hospital, infant in hand. More often than not, mother and family are left on their own until a cursory 15-minute visit with a healthcare provider several weeks later. During long gaps between checkups, mothers experience deep worry for their infants. They struggle with rapidly accelerated responsibilities, extreme sleep deprivation, and relentless pressure to return to work. And all while recovering from pregnancy and adjusting to parenthood — a transition that marks one of life’s greatest physiological endurance tests. Too often, this experience is isolating, disempowering, and mortally dangerous.  And over time, these risks are getting increasingly severe.
